In addition to what wine said, if deployed to a combat zone you are not supposed to wear any badges or tabs except for those worn as part of a unit patch (ie airbone or mountain tabs). You are also not supposed to wear any kind of pin on insignia while in the field.
The only way you would ever lose it after you have earned it is if you are ever on jump status and refuse to jump. The first O-6 in your chain of command can revoke your award of the badge.

It is a permanent award yes.
