I was out for 5 days sick, and missed a couple days a month ago too. I'm better now and will be returning to work monday. I was told by my manager that h r would like me to fill out fmla paperwork?? Is this something they can require of me? And its scaring me, like I have to worry about my job now?

They can require you to take the FMLA or lay you off, they have to know when an employee is going to be on the job or not.~~
I wouldn't be scared. If they are offering you FMLA paperwork it is because you have missed a significant amount of time for an illness. I think if you are out for 3 consecutive days for an illness then it may be perceived as a serious illness which FMLA would kick in, if applicable. A lot of employees do not understand that FMLA is a job-protected leave; it's a benefit to employees. Under approved FMLA coverage, employers are obligated to hold an employee's position until he/she returns to work. As long as an employee doesn't exhaust FMLA coverage during the absence, he/she must be reinstated into the same position they were in when the leave began, or an equivalent position in some cases (equivalent pay, hours, schedule, etc.).
Yes any time you are out sick 3 or more days they should ask for it. It's to your advantage to do it as time used under FMLA does not have a negative impact on your attendance record.
The business employer "might" furnish "non-compulsory" kinds, in accordance to the guidelines. consequently, the business employer isn't required to furnish to you, or perhaps use, any kinds. Since they have no legal obligation to give you any sick time OTHER than FMLA, they certainly can require it, if it applies.

Yes they can and it is for your protection. It covers missed time which then cannot be held against you later.
if you are out of sick time and vacation time they can.
