The second piercing part definitely hurts more than the first (probably because you expected the first one to hurt).
The piercing is performed with 2 hollow needles generally. Unless your piercer likes to use one, which gives it a better alignment. Then the barbell will follow through the needle.
I believe you should have a choice if you want two seperate barbells or just one whole one for the piercing (since it IS two piercings).
It is not reallie more prone to infections HOWEVER, people think that getting "the bump" is an infection. It is not an infection. It is just hypertrophic scaring and will eventually go away on its own.
Serious risks include infections, that is most likely if you go to a shady place to get it pierced. Probably the worst case is that your ear rejects the piercing, in which case, the piercing slowly moves towards the skin and eventually fall of your ear.
I have one. I recommend it.
Healing process takes a while though. I've had mine for over a year and it is stil not fully healed (meaning it gets crusty once in a while, but crust means its healing).

i had it, it was probably a 0 on the pain scale, but i had a insane adrenaline rush so i felt nothing. expect the worst and youll say to yourself, thats it?
they take one needle and put it in a jelly like formula. then they mark ur ear and ask if your satisfied. then they take the needle, pop it in from near ur head, then while its in the first hole, they stick it in the other part creating the second hole. some piercers use 2 needles but i usually always see one. then they slide in the bar. afterwards it throbs and stings for a few hours
it can be prone to infections because your taking care of TWO piercings, not just one. and it will get caught on alot of stuff. like hair, clothes etc.
there isnt really serious risks if you take care of it and get it done at a CLEAN REPUTABLE place..
i recommend it bc i LOVE mine, it took like a year to heal. its beauuuutiful
