Almost all doctors do things differently; but generally speaking a nurse usually takes your weight, height, blood pressure, and temperature. A doctor then asks you some questions (she/he will ask about your period, if you're sexually active, if you smoke or drink, etc.), then they'll checks your reflects (takes a few seconds and doesn't hurts), they'll listen to your heart and lungs, check your skin for unusual marks (signs of skin cancer), and look in your eyes, nose, and ears. Some times they also check your balance by having you walk in a line heel to toe. The doctor might also bend you over and run their hand down your spin to make sure that your spine is straight.
Yes, it's probably a good idea to tell her about the itching. The doctor will either discuss symptoms, talk about common reasons and prevention for itching, or do a pelvic exam. Over all it is quick and painless, good luck because I remember how nervous I felt too. :)
Do not bring it up if you don't want a pelvic exam!
A physical is for sports;to make sure you are healthy enough to play a sport.
Since you have your period, they may or may not ask you. If they do, they will probably give you a smal shot in your finger to make it less heavy.
Things they do at a physical:
-hearing/sight
-heart (make sure it sounds right)
- eyes (make sure nothing odd is in the eye)
- maybe a pelvic exam (you'd want a female nurse for this.)
- stomach (to make sure there's no odd lumps in your belly
- thyroids (in your neck, they make sure they're not swelling)

Usually they won't do a pelvic exam until you are sexually active. And when the physical is done, they will ask you if you have any questions. And by all means, if you have questions, ASK THEM! Those nurses are there to help you! It could be a question about acne. They would prescribe you a medication like Xout, or Epiduo.
I'm 14 and I'm gonna be a sophomore. :)
They just check your ears, mouth, eyes, press on your abdomen and ask you to bend over to see if your spine is in line (at least at my physicals it's done that way). Also they weigh you and probably will prick your finger. I just changed to a different office this year so now they also feel your breasts to make sure everything is normal.
I would mention it if I were you. If she thinks it could be something, she'll send you to see a gynecologist.
