My friend went to mexico for a month this summer and came back with one of those-- and i had the same question! It's really quite simple, you just work the plastic ring off of the bottle and then once it's off you stretch it out until it can fit around your wrist. At least this is how she described it, i'm sure there are several other ways to do this though.
