My friend had that problem with Black Ops and he had to replace the CD... He said it probably happened cuz he restarted Xbox too many times in a certain period of time with the game inside.

is it a legal copy thats one reason then try downloading it to your hardive another thing clean the disc also make a copy of the game and it should work
