To transfer bank accounts, take granny on an outing to the bank. They will set up any accounts however she directs. If there's real estate, she'll need to have a lawyer prepare deed(s). This is a legal document; don't try to do it yourself with a fill in the blanks form.

If you are trying to reduce assets so that she qualifies for government programs the starting point is purchasing anything that she needs. She should buy a pre paid funeral and burial. If she will need any home repairs such as a new roof now is the time. If she needs remodeling for handicapped accessibility then do it. If she needs any new furniture or a hospital bed etc. then take care of it. If she needs a car so that others can transport her then buying a car would also be a valid expense. After all of her needs are satisfied you will need an estate attorney to advise you as to how to make the rest of it go away.
You will need a probate lawyer
