I chose to delete all temporary internet files through the IE options. But there are still 272 items remaining in that folder, inlcuding some very mundane items like text files nad a lot of cookies. Why isn't everything getting deleted? What can I safely delete? I had a javascript virus (JS/blacole/W.) detected and removed but I'd feel better about cleaning out this folder. There are still a number of java items left in there.

Sunday, as I understand it, certain files are sorta on 'standby' awaiting access by Windows. Those files are 'in use' and can't be modified so long as Windows is running.
There's a proggie called CCleaner designed to deal with such files. It's fully configurable to handle things like index .dat files as well.

It does it's thing (if needed)on bootup before Windows ever starts. Works good for me.
Another option is to go to IE, Tools, Internet Options, Advanced. Scroll to the bottom of the page and place a check mark next to "Empty temporary internet files folder when browser is closed", Click Ok.
It does not clear out the index.dat file but if you have nothing to hide then don't even waste time thinking about it. If the feds need the info they can get it by other means anyway.

This should be a routine opperation. That it isnt, suggests something doesnt want to be deleted. I'm reasonably experienced, and have never encountered this as a topic/problem in a forum to do with regular microsoft problems. I have had viruses, and all sorts, but not this, so it just may be a legit conflict, but certainly viruses to wiered things. My best suggestion, is a re format, and re install windows, if your competent, have the disks, and can back up personal files. If not, it may be a long slug with antivirus, But dont download any free scanners etc, that may appear. Obviously, do not do any pc banking or online purchases until its sorted. sorry cant be of more help, but hope its helpful advice in the mean time.
Not sure why those files are not being removed, but here are some programs that can do it for you ... Even if you set your browser to "remember for x days" ... if you hit delete, they SHOULD delete, so I can explain that ... POSSIBLY the infection still being there maybe (I admit i am guessing)
A) CCleaner - ( http://www.ccleaner.com/ ) - One of the best and most well known programs for this purpose ... it also has the options to remove custom (which ones you select) restore points and to clean the registry but the most powerful part of the program is the temp file removal .. .If you only use one program, I suggest CCleaner. It has support for the latest IE, Opera, Firefox, and Chrome Browsers among other programs and temp file locations
B) CCEnhancer - ( http://majorgeeks.com/CCEnhancer_d6547.html ) This little app is for CCleaner and adding many more programs to it's "Database" ... It doesn't remove anything (CCleaner) unless you TELL IT to. ... you run this from time to time updating like a malware or antivirus scanner
C) Bleachbit ( http://bleachbit.sourceforge.net/ ) - supposedly gets some of the LSO persistent Cookies in various locations that some other programs (at the time I am typing this) can not

Also if it is the infection, I would suggest getting (or switching to) a GOOD AV such as Avast or Avira
Both of these top two scanners have similar (Great) detection rates and are “neck and neck” as the best Free anti-virus if you ask me.

Go to start and then all programs and then accessories and then system tools and run disk cleanup. It will scan how many files you dont need and when the scanning has completed it will say temporary internet files make sure you tick every box and then click ok and let it cleanup and then they will be deleted.
Default settings will have Temporary Internet Files not errasable before 24 hours are up.....you can change that in the advance settings............or simply use this free program and delete all temp files....
