Before the referee got to 10 Cena was attacked from behind by Ryback. Why didn't Cena win by DQ like what usually would have happened in that scenario?

Here's the real story. The writers came up with the idea that Cena would leave the ring when Ryback showed up and the referee would count him out. Then John Cena and company acted it out according to the script. That's how he lost.
Because cena intentially left ring therefore lost by count out. Remember WWE is all scripted and is designed for fans to go along with whats happening even though its not always consistently correct

Because Ryback didn't hit Cena until the referee got to 10.
Probably because the referee didn't see Ryback attack John Cena.
Cena left the ring on his own terms and the ref had already begun counting. We're seeing Axel fire through WWE. He's beat "Sin Cara" twice now.
