Buy him some socks/under ware and wrap them in a large box (with a brick to add weight, and some marbles in a plastic jar to make sounds). Wrap it ahead of time so that he will see it for several days before his birthday. Tell him that this is his only present from you. Take lots of pictures when he opens his present.
Wake him up one day telling him he's late for school, rush him (when it's really like saturday, or anyday really since it's still summer for the kids). Make him put on his new shoes, and new backpack. Then tell him how late he is, he's gonna get in trouble on his first day of school. Then take him to his (surprise) birthday party.---Just an idea. But i have woken up like on a saturday at 9 am & totally freaked cuz i was late, then realized it was saturday...LOL.

Do something where you pretend no one shows up to his party, and then have everyone jump out after a bit.
Or have everyone ther and pretend the whole party that he gets no presents and no cake. Then ovibously give it to him.
