If you have done this tour, how was it? Do you have to book before you get to your resort? or can you do it after you get there? And one last question; My wife will be VERY pregnant (7 months or so) while we are there, should we just forget this tour or will there be some things she would be able to do?

This tour is just a smaller version of a group tour. Basically you can choose from different itineraries and days that they are offered. Their offer is to pick you up at your hotel, private more expensive, a small group less. And whisk you off to several destinations of your choice. Directly to an archaeological site or if you prefer stops where you want or suggested by your driver/guide. You may book once you are here. I know of several English speaking taxi drivers who offer this service, at a fraction of the cost. Got another question just ask or contact me directly.
My experience in the health care system for many years tells me that at 7 months a woman develops frequency, which means frequent trips to the wash room because of the pressure put on the bladder by the baby. If this is happening, I suggest forgoing tours unless you get a tour bus with a toilet on board. I'm not trying to scare you, but it could be very uncomfortable for your wife. If you went into Playa del Carmen, which is quit interesting along 5th avenue. All walkable, full of stores, restaurants, washrooms and a block away from a very nice beach, might be better and easier.
I would not make plans and see how she is feeling day by day.
