If a supervisor knowingly falsified a verbal report regarding my performance in order to threaten my future advancement in my career, is there a way to file a grievance? I have documentation that verifies that the claim against me was false in the form of witness statements. Attempts to further communicate with her have been ignored. I love my job and am qualified for the position I was in line to receive.

your supervisors OPINION and PERCEPTION of you is all that matters, you are going to find yourself on the wrong end of the stick if you continue, HR cant help you all you can do is take his criticism and continue on
Truth is if she is the kind of tool she appears to be it will be a lot easier for the bosses to decide to fire you and live with her because you are powerless and she seems to know where the bodies are buried.
There is nothing more worthless than a tattle tale report to HR and it will only get you a rep as a trouble maker. Either keep your head down and find a sneaky way to get her job or just do the job you have.
First stop should be the Human Resources department at your workplace.They are uniquely qualified to reach an unbiased verdict.
Report your situation to the Human Resources or upper management people at your place of employment.

go to the top boss of the firm and give him-her a dozen recommendations for
growth for the company--to include cost savings, and new sales.
if you can't, be prepared to be fired.
[sorry, but this is my strength and unless you can show the boss that you are
worth 10x what you are being paid, he-she will not care about any reports.
You would have to prove they knowingly lied. Witness statements are no more proof they lied than their statement is proof you did it.
